RS009 registration card: what it contains and how it can go digital
The registration card is the statutory document on which an approved Belgian driving school records every lesson given to each student, with the instructor's signature. Model RS009 is annex 9 to the ministerial decree of 30 March 2005. The card may be kept digitally, provided it is always accessible and can be printed when an inspector asks for it.

What is the registration card?
A driving school draws up a registration card (inschrijvingskaart in Dutch, carte d'inscription in French) for every student. This comes from article 23, § 1, of the royal decree of 11 May 2004 on the approval of driving schools. The card states the student's identity, enrolment number and enrolment date, and has a box for each lesson the school gives. In the trade it is also known as the yellow card.
The model itself sits in a separate text: article 9 of the ministerial decree of 30 March 2005 refers to annex 9. The Dutch version of that annex carries the code RS009, the French version AE009. They are the same form.
For each student, the card shows which lessons were given, when and by which instructor. The officials who inspect driving schools may examine it (article 39). The certificate stating the number of hours followed is a separate document.
What is on the card?
The model has three pages: one with the details of the school, the training and the student, one for theory lessons and one for practical lessons. These are the fields of the model as published by the Brussels Region, in plain English.
| Part | Fields |
|---|---|
| School | Name of the driving school (stamp), the school's approval number and the registration number of the establishment unit |
| Training | Type of instruction, any earlier training document held by the person, category, enrolment number, enrolment date |
| Student | Surname, first names, address, postcode, municipality, place and date of birth |
| Signatures | Name and original signature of the director (when delegated: the director's name, plus the name and signature of the delegated instructor), the student's signature at the end of the cycle |
| Lessons | Separate tables for theory and practical lessons: for each lesson the date, start time, end time, and the instructor's name and signature, with the student's signature at the bottom |
How is the card filled in?
The model and article 23 set out the method. On paper it works like this:
At the start of the lesson
The instructor notes their name, the date and the start time.
At the end of the lesson
The instructor notes the end time and signs.
At the end of the lesson cycle
The student signs the card and receives a copy.
How long do you keep the card?
The retention period is twelve months (article 23, § 3). The card belongs to a set of documents from the same article, each with its own model.
| Document | Model | Article 23 | Retention |
|---|---|---|---|
| Registration card | RS009 / AE009 | § 1 | 12 months |
| Attendance list for theory lessons | RS010 / AE010 | § 2 | 12 months |
| Instructor's daily sheet | RS011 / AE011 | § 3 | 12 months |
| Annual register per establishment unit | RS012 / AE012 | § 4 | 36 months |
Can the registration card be digital?
Yes. Under article 23, § 5, these documents may be replaced by a data carrier for computer processing. Two conditions apply: the data must be fully and continuously accessible, and it must be possible to reproduce them on paper in an understandable form when the inspecting officials ask. For a driving school, that comes down to this:
- Every student's card can be called up at any moment, including during an inspection at the establishment unit.
- You can print it legibly on request. The safest approach is to use the same fields as the model.
- The student still receives a copy at the end of the cycle.
The model asks for the director's name and original signature, and article 23 does not say what form a signature must take digitally. If you are unsure whether your way of working is sufficient, put it to the authority in your Region.
What is changing in Flanders?
Article 23 currently treats the registration card the same way in all three Regions. In Flanders, the decree of 15 March 2024 on professional driving instruction (in Dutch) enters into force on 31 December 2026 at the latest. Its article 19 allows the Flemish Government to regulate the administrative obligations of approved driving schools. As of 4 November 2026 we had not found a new model. Schools in Flanders should therefore check with the Departement MOW what applies at that point.
This guide gives general information and is not legal advice. The competent authority is the service of the Region where your driving school is established: the Departement Mobiliteit en Openbare Werken in Flanders, SPW Mobilité et Infrastructures in Wallonia and Brussels Mobility in Brussels.
The registration card in BIQSDRIVE
In BIQSDRIVE you create the card for an enrolment in one click, as a PDF, with the fields and order of model RS009. Practical lessons come from the lesson reports that instructors complete in their smartphone environment, theory lessons from the courses the student followed. The full flow is described on Digital driving school.
- Practical lessons appear with date, times and instructor, marked 'Signed digitally' with the time of signing. Theory lessons followed, and practical lessons completed without the instructor's signature, appear as 'Confirmed digitally'.
- The student's signature comes from their latest lesson report. The director's is printed if it has been stored, otherwise the box stays empty for a handwritten signature.
- The student downloads the card themselves in the student portal, in their own language: Dutch, French, German or English.
- Every version is kept for twelve months and you can print it at any time.
FAQ
Frequently asked questions about the registration card
Is the registration card compulsory?
Yes. Article 23, § 1, of the royal decree of 11 May 2004 requires one for every student, for theory and practical lessons.
What is the difference between RS009 and AE009?
There is no difference in content. RS009 is the Dutch version of annex 9, AE009 the French version.
Where can I find the official model?
The Justel database does not include the forms and refers to the Belgian Official Gazette of 29 April 2005. The Brussels Region's page on driving schools offers the model as a PDF: RS009 and AE009.
What if a student changes driving school?
The school issues a certificate stating the number of hours followed (article 23, § 6). That certificate is a separate document with its own model.
